    Wedding/Reception Request Form
    You must be at least 21 years old in order to make this request.

    When you submit this form, the following request(s) will be submitted to our church office:
       1. Use of the facility for your wedding ceremony
       2. Use of the facility for your wedding reception
       3. Use of the facility for your wedding rehearsal and rehearsal dinner  
     
    If your wedding will not be on our church campus, but you would like to add your event to our church calendar, please use the standard Activity Request Form.

    After you submit this form, our church office will use the contact information you provided to get back with you to let you know if your event is approved, if there are any conflicts, or if any additional information/arrangements are required. Please allow up to 1 week for processing.

    Terms / Agreements for Facility Use
    • I have noted that weddings ceremonies at Western Hills cannot be scheduled after 2:00pm on Saturdays or at any point on Sundays.
    • I will provide a team of volunteers to help with the following tasks after the event:
      1. Leave the room(s) I use free of trash and clutter.
      2. Work in harmony with the church wedding coordinator to return all items back to their proper place.
    • I will not use more equipment or space than requested on this form unless I receive additional authorization from the church wedding coordinator. 
    • If I have any questions or problems, I will contact the church wedding coordinator.
    • I will submit a non-refundable deposit of $100 as soon as I receive confirmation for this request. I recognize my wedding will not be added to the church calendar until this deposit is paid. The deposit will be added towards my total balance.
    • Non-Members Only: I will submit an additional $300 deposit as soon as my request is approved if I am using the Family Center. I recognize my wedding will not be added to the church calendar until this deposit is paid. The deposit will be added towards my total balance.
    • I will submit all outstanding payments at least two weeks before the event. I understand that if the balance is not received by this point I may lose my reservation.
    • I am aware of and agree to the additional procedures and policies in the Western Hills’ wedding packet:
      1. Procedures and Policies for Members (ADD LINK)
      2. Procedures and Policies for Non-Members (ADD LINK)

    This request form is not a contract. However, if you have questions about building and/or cleaning fees, feel free to contact our church office at 405-634-1454.



    Logistics: Opening Up, Locking Up, and Cleaning Up

    Are you connected with anyone who might be able to complete the following tasks for your event?
    Is there anyone connected with your event who might be able to unlock the building for you?
    Is there anyone connected with your event who might be able to lock the building for you?
    Who is the responsible person for cleaning up? (If a deposit is required, the person who places the deposit is often the one ultimately responsible.)
    Office Hours: If your event is during normal office hours (Mon-Thur, 9am-4pm) and not on a holiday then you will not need anyone to lock or unlock the building for you.
    Please Note: Our leadership will ultimately determine who will unlock, lock, and clean up. It is helpful to receive recommendations from the person hosting the event.

    Terms / Agreements for Facility Use
    I agree to leave the room(s) I use free of trash and clutter, and to make sure the furniture is arranged in an orderly manner. If the room was set up in a particular way before I used it, I agree to make sure to leave it as I found it.

    I will not use more equipment or space than requested on this form unless I receive additional authorization.

    If I have any questions or problems, I will contact the church office or a contact person they assign ASAP.
